Registration FOrm

Fill out the form below or we have paper registration forms at all 3 entrances of the church to register your child for Religious Education. You can register up to 2 children per online form. Please pay in person at the office, Monday-Thursday 9am-2pm.

One registration form necessary per family
Fees:
- $75 per family
- +$25 per child for sacramental years (1st Communion & Comfirmation Years 1 & 2
Must be registered members of St. Therese Parish
Photocopy of Baptismal Certificate must accompany all Sacramental Year Registrations
Tuition due in full at time of registration
Virtus Training=Discount
