Confirmation Signup Form (1 &2)

With this confirmation year form, I am requesting to be enrolled in the Two-Year Confirmation Preparation Program of St. Therese Parish. I will prayerfully and earnestly fulfill all requirements which are stated below. I pledge to grow in my knowledge and practice of my faith with the help of my parents, sponsor, and parish family.

  • Attend Mass each week

  • Attend Confirmation Classes each week

  • Attend Confirmation Retreat (Second year)

  • Participate in Class Service Project (One per year)

  • Assist with at least 2 Parish Activities per year

  • Complete 12 Volunteer Hours per year

  • Write a Letter to Father William requesting the Sacrament of Confirmation (Second year)

  • Write a Biographical Summary on the saint selected as the Confirmation Name (Second year)
